It has come to the attention of Gateway Rehabilitation Center (“Gateway Rehab”) that a data security incident may have affected the personal and/or protected health information of some current and former patients.

Gateway Rehab is a non-profit organisation that provides drug and alcohol rehabilitation services throughout the greater Pittsburg area. Gateway Rehab has informed those who may be affected and made available resources to help them.

Gateway Rehab found out on June 13th, 2022 that it had suffered an incident that prevented access to some of its systems. After discovering that these systems were not properly secured, Gateway Rehab quickly launched an investigation. Gateway Rehab hired third-party digital forensics and incident response specialists to investigate the breach and locate any potentially compromised data. On July 8, 2022, after completing its investigation, Gateway Rehab confirmed that data possibly containing personal and/or protected health information may have been impacted and began a thorough review process to determine the exact nature of the information and the individuals involved. As of the conclusion of the investigation on September 21, 2022, it was determined that the data breach may have included the personal and/or protected health information of current and former patients of Gateway Rehab. All potentially affected individuals were notified by letter from Gateway Rehab on November 18, 2022, and were provided with resources to help them maintain the security of their personal data.

Individuals’ names, dates of birth, Social Security numbers, driver’s licence or state ID numbers, financial account and/or payment card numbers, medical information, and health insurance details may have been compromised. There is no indication that any of this data has been misused, according to Gateway Rehab.
